Welcome New Clients!

Thanks for considering ACAH for your pet’s healthcare needs.  We’d love to meet you!

We want to be as prepared as possible for your first visit, so we do ask that you do call us at 573-875-0907 to schedule your initial visit.  We will need additional info about you and your pet and discuss previous history for your pet.  We do require a $20 deposit to schedule your initial visit and can arrange that by phone for you as well.  Once we have your account established, you will have access to use our PetDesk app or website to request appointments, refills, or leave messages for your pet’s doctor.

If your pet has had care elsewhere, we’d like to collect previous records whenever possible.  This helps our doctor learn more about your pet before your appointment and helps ensure that treatments/testing are not unnecessarily repeated for your pet.

  • If you need any assistance in obtaining your pet’s previous records, we are happy to reach out to your previous clinic for you. Please let us know the location and name on previous records.
  • If you have printed records to share, they may be sent to or texted to 573-875-0907. Please include your name and appointment date so that we may match your records to your scheduled appointment.
  • If you prefer to request records yourself or your previous clinic requires a client consent to release records, please request records be sent to

We know filling out forms with a leash or carrier in your hands can be challenging! Our new client form is available below and may be filled out at your convenience and emailed to us prior to your appointment.
